7. PSA marriage certificate latest (original and photocopy)
8. Tour Itinerary, Hotel booking, Roundtrip Airfare, Travel Insurance
Financial Capacity:
1. If employed – (Original)- Certificate of Employment
(status, date hired, compensation with HR contact and email address).
2. If self-employed – (Photocopy)- Business permits- DTI, Mayors, BIR, ITR
3. Original Bank certificate with embassy address
(account type, current balance, account opening date, and ADB)
4. Original Bank statement of account- For the last 6 months
5. Photocopy of ITR- income tax return or form 2316
Student:
11. School ID (photocopy)
12. Original School certificate of registration/enrollment
Sponsorship:
13. Sponsor Guarantee letter and Affidavit of Support
14. Proof of relationship
15. Sponsor will provide the above financial requirements
